By accessing our website or booking an appointment with us, you agree to comply with these terms:

Terms of Treatment:

I understand that I have the right to be informed of the procedure, any alternative options, and the risks and benefits of IV therapy. Procedures will not be performed until I have the opportunity to give my informed consent, except in the case of an emergency.

My booking confirmation acknowledges that:

  1. This procedure involves inserting a needle into the vein and injecting a prescribed solution.
  2. Alternatives to IV therapy include, but are not limited to, oral supplementation.
  3. The potential risks and complications of IV therapy include, but are not limited to:
    • Occasionally: Discomfort, bruising and pain at the injection site.
    • Rarely: phlebitis/thrombophlebitis, hypervolemia, extravasation, flare reactions, air embolism, cellulitis, infection/sepsis, metabolic disturbances, and injury.
    • Extremely rarely: Severe allergic reaction, anaphylaxis, cardiac arrest, and death.

Benefits of IV therapy include:

  1. Injectables are not affected by stomach, or intestinal absorption disturbances
  2. The total amount of infusion is available to the tissues.
  3. Nutrients are forced into cells by means of a high concentration gradient.
  4. Higher doses of nutrients can be given than is possible by oral consumption.

I am aware that unforeseeable complications could occur, and I do not expect IV Wellness to anticipate or explain all possible complications. I rely on IV Wellness to exercise judgement during the course of my treatment.

I understand the risks and benefits of the procedure and have had the opportunity to have all of my questions answered. I understand that I have the right to consent or refuse any proposed treatment at any time.

My booking confirmation affirms that I have given consent to IV therapy with IV Wellness understand that all nutrient infusions are considered investigational/experimental and are not considered standard of care.

My booking confirmation confirms that:

  1. I understand the information provided on this form and consent to treatment.
  2. The procedure(s) set forth above has been adequately explained.
  3. I have received all the information and explanation I desire pertaining to the procedure.
  4. I authorise and consent to the procedure(s).
  5. I understand that IV therapies are experimental.

High-Quality Products and IV League Approval:

We are committed to using only high-quality products approved by IV League. Our IV fluids are sourced from reputable suppliers, and all products used meet the standards set by IV League.

Experienced and Certified Nurses:

All our registered nurses are experienced and trained by IV League. They are certified to provide IV therapy treatments, ensuring that you receive professional and safe care.

Storage and Handling:

Our IV fluid bags are exclusively Hartman’s and are stored in dry places within their specified expiry dates. Vitamins are refrigerated as required, and when taken to a treatment, they adhere to the recommended procedures, with a maximum timeframe of 6 hours without refrigeration.

IV Therapy as Supportive and Proactive Treatment:

It’s important to note that IV therapy is not a substitute for general health and wellness practices. Our services are designed to be supportive and proactive, enhancing your overall health and well-being.

Health Concerns and Professional Advice:

If you have health concerns or questions about our IV vitamin treatment, please reach out to us. We work closely with IV League and our consulting Doctor to provide health advice tailored to your individual needs.

Consultation with Your Doctor:

For your safety and peace of mind, we recommend consulting with your primary healthcare provider before booking our services. They can provide personalised advice based on your medical history and conditions.

If you experience any discomfort or have concerns after the treatment, please inform us promptly. Your well-being is our priority, and we will address any issues to ensure your satisfaction.

Vitamin Deficiency Testing:

We offer vitamin deficiency tests to identify any potential deficiencies in your system. This information helps us tailor our treatments to meet your specific needs.

Reservation Changes and Cancellations:
